We welcome all men�s and women�s club lacrosse teams to participate, in
this, our inaugural year.  We have interest from teams in Japan, Korea,
California, Utah, Hawaii, and various other parts of the United States
and Canada.  We are looking forward to having a strong group of teams to
participate in this round robin event.  Each women�s team is guaranteed
5 games and each men�s team is guaranteed 7 games.  We anticipate that
these games will be competitive and exciting.

The entry fee is $750 per team and this, non-refundable amount,  must be
in by June 1st to guarantee a spot.  Checks should be made to Victoria
Summer Slam.  Women�s teams will also need to provide the names of two
team members that could referee, if need be.  Once we have received your
checks, rosters, and waiver forms we will be sending you an information
packet which will have hotel suggestions, tournament rules, game
schedule, sponsor lists, tourism packets, etc.  This is a long weekend
in Canada, and therefore, we suggest you book your hotel rooms early.

We have planned  evening activities on Saturday and Sunday night.  There
will be an on-site beer garden as well as lacrosse/athletic vendors.  We
will also have food vendors at the fields.  We are  working on getting
discounted whale watching events and discounted hotel rooms.

The games will be played from approximately 9am-5pm, with playoff games
on Sunday afternoon.  Prize money will be awarded, in cash, following
the championship games.

 In consideration of the acceptance of this entry, and intending to be
legally bound, I hereby waive and release for myself, my heirs,
executors, administrators, and assigns and and all rights and claims for
damages I  may have against the Victoria Summer Slam Tournament and its
planning committee,  sponsors, officials, employees, agents, and
representatives for personal injury or property damage or loss, whether
such loss results from my own negligence or that of the participants  or
from any other cause suffered during the Victoria Summer Slam
Tournament, held in Victoria, BC on August 3 and 4, 2002.

I acknowledge that my voluntary participation in this tournament
involves inherent hazards and risk of personal injuries such as, but not
limited to: paralysis, brain damage, loss of vision or limb function,
permanent scarring and/or death and I assume such hazards and risks
concerning my participation.  I represent that I have the necessary
physical abilities and conditioning to safely participate in this sport.

I HAVE READ THIS DOCUMENT, UNDERSTAND IT TO BE A BINDING CONTRACT, AND
